摘要

Across the road from the Accademia Nazionale Dei Lincei in Rome is the Villa Farnesina, completed in 1509 by the renowned architect Baldas-sarre Peruzzi. In the garden of the Villa is a marble plaque that reads:
Quisquis huc accedis: quod tibi horridum videtur mihi amoenum est; si placet, maneas, si taedet abeas, utrumque gratum.
The words on this plaque give warning to those entering the Villa Farnesina garden. Translation: "Whoever enters here: What seems horrid to you is pleasant to me. If you like it, stay; if it bores you, go away; both are equally pleasing to me." The words could also be provided somewhat irreverently by this editor to those opening the pages of this 2014 edition of Year Book of Pediatrics, the editor's 36~th, and last.
A bit of history on the transitions of prior editors of Year Book of Pediatrics seems in order. In a publisher's note to the 1979 edition of the Year Book, it was remarked that Dr Sydney Gellis had relinquished his role as editor of the Year Book after 27 years. Dr Gellis was known for commentaries that were thoughtful, instructive, and often humorous. He was always able to keep new knowledge in perspective. Previous editions of the Year Book had been edited by individuals equally prominent in medicine and pediatrics: Drs Isaac Abt, Henry Poncher, and Julius Richmond. With the need to pass the editorial torch at that time, the publishers looked to someone with similar pluck to Dr Gellis. The word "pluck" is little used today, but in earlier years it was the preferred term for heroes and heroines who displayed a combination of resourcefulness, dash, and spirited courage to take on challenges that others often neglect. They did not have to look very far to find someone with pluck. There was Dr Frank Oski, unrivaled in his acumen as a pediatrician with trenchant wit. At the time, Frank was professor and chair of the Department of Pediatrics at the State University of New York Medical School in Syracuse. He was a world class generalist and a subspecialist in pediat-ric hematology. In his first introduction to the Year Book, Frank wrote that he had gladly accepted his new role, but with some trepidation. He quoted the Arabic proverb: "When fate arrives the physician becomes a fool." He was referring to the fact that it was a next-to-impossible task to take on the solo editorship of a comprehensive annual review of pediatrics. It was Frank's intent to do exactly that, at least until the quickly looming deadline for the 1979 Year Book manuscript necessitated a different strategy. That is how I became an "accidental" associate editor of the 1979 Year Book. I was less than a handful of years out of a pediatric fellowship and somewhat of an Oski "protege" (there were actually quite a number). Frank threw in the towel at the solo writing and asked if I would be willing to finish 4 of the 19 chapters of the 1979 Year Book. He didn't have to ask twice. I jumped at the chance. From the 1980 edition onward we were equally splitting the writing and were co-editors until Frank's resignation in 1991. It's been a solo effort since then.
It has been an honor to prepare the Year Book of Pediatrics, and while the honor has been executed with continuing irreverence from time to time, it's hard to match the pluck of Drs Oski and Gellis. As an example, in his very first attempt at editing the 1979 Year Book, Frank wrote a commentary on the topic of smoking in pregnancy. The commentary (p.71) read:
"Pregnant women should not drink. Pregnant women should not smoke. If you give up drinking, it should be easier to give up smoking. Pregnant women should not take drugs. Shouldn't we ask fathers to make the same sacrifices as well?
"Every time I hear about the dangers of drinking and smoking, I am reminded of the following story of two old men: First man: 'I have never made love to women, I have never smoked and I have never had a drink. Tomorrow I am going to celebrate my 90t birthday. Second man: 'How?'"
So it is not with a joke, but with some irreverence and a modest degree of celebratory crapulence,* that I close this episode in the history of editing the Year Book of Pediatrics and turn over the honor to another. Last episodes are often poignant moments in history. The very last TV episode of St Elsewhere (younger readers will need to check with Wikipedia on the origins of a fictional hospital in Boston called St Eligius) is a favorite and ended with the tale of a rather corpulent female opera singer, in full Wagnerian regalia, including an armored breast plate and helmet with horns. She was experiencing aphonia, secondary to a bad case of laryngitis. In the closing scene of the series, Howie Mandel (in his days with hair and playing an intern at St Eligius) looked straight into the cameras and uttered the not-so-PC Yogi Berra-ism: "It ain't over till the fat lady sings."
She did ... camera fade. So does this introduction and this era. ...Adipem domina cecinisse
