Total spending on healthcare, including both public and private, increased rapidly in the pre-crisis years, rising from 6.1% of GDP in 2000 to 9.9% in 2009. The percentage has since fallen further, to stand at an estimated 8.8% of GDP in 2013. This is broadly in line with levels in Spain and Italy, but well below those in France, Germany and the Netherlands (11-12% of GDP).
Since 2009 healthcare spending per head has contracted sharply, falling to about €3,300 in 2011, almost 15% lower than at its peak in 2008, reflecting severe fiscal pressures on the public healthcare budget and reduced consumer spending as a result of high unemployment and lower incomes.
We forecast total health spending as a percentage of GDP to remain stable at 8.8% of GDP up to 2019, with total spending growth averaging 3.2% a year in US dollar terms in 2015-19 (or 3.5-4% a year for spending per head in euro terms). Nevertheless, the continued spending crunch raises the prospect of a further reduction in capacity and lower levels of service provision.
